Challenging youth to pursue a life of Spirit-filled holiness through a personal relationship with Jesus Christ within the community of the Catholic Church.

Below are the generic forms to register for the overall youth program and all other youth events and retreats.

To register for the overall Youth Program, return your completed form and payment to the parish office. See page 2 of the form for volunteer opportunities. To download the 2008-2009 Registration Form click here.

A Youth Parent Permission Slip is required for each youth event off-site. Please register by the designated deadline by returning the form to the Youth Office. To download the permission slip click here.

An Adult Liability Waiver is required for all adults helping with any off-site youth event. To download the Adult Liability Waiver click here.

If your child will need to take any medications during a youth event, a completed Prescription Drug and Medicine Authorization form is required. To download the form click here.
